               Role: Hub Home Provider
               Location: Perth (North Suburbs)
               Employment Type: Maximum Term, Part Time - ending 31 December 2027.

As a Hub Home Provider, you will be part of Life Without Barriers’ Mockingbird Family Constellation, providing practical support to Satellite Families and the children and young people in their care. You will provide a safe and welcoming Hub Home where foster and kinship carers can connect, share experiences and support one another.

You will build trusted relationships with carers and families, helping them navigate challenges, access support and feel connected to their wider community. Through this role, you will help strengthen the capacity of foster and kinship carers and promote positive outcomes for children and young people.

Key Responsibilities

• Coordinate and host monthly Constellation meetings, quarterly social outings and activities for carers, children and young people.
• Provide regular support to Satellite Families and children and young people, including assistance with placement challenges and crisis situations.
• Connect families with education, mental health and community services to support positive outcomes.
• Host case planning, care team and family meetings at the Hub Home where appropriate.
• Build strong relationships with carers, children and young people and provide a welcoming and supportive Hub Home environment.
• Maintain accurate records and work collaboratively with carers, Life Without Barriers staff and community services.

Skills & Experience

• Experience as a foster or kinship carer (5+ years preferred), or relevant experience in Child Protection or Out of Home Care.
• Ability to meet the requirements to obtain and maintain authorisation or registration as a foster carer under the relevant jurisdiction.
• Strong relationship building and communication skills, with the ability to work positively with carers, children, young people and other stakeholders.
• Ability to support children and young people and respond appropriately to placement challenges and changing needs.
• Current Provide First Aid (HLTAID0011) Certificate.
• Current Driver’s Licence.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and any relevant stated based checks.
