About the Role
As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services on the Sunshine Coast. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
You will play a key role in program coordination by evaluating, refining, and managing operational and administrative systems. Your focus will be on providing outstanding service that ensures clients and their families feel supported, while contributing to the ongoing growth of our Aged Care services.
This is a permanent full time role based on the Sunshine Coast, travel will be required to support our clients from Caloundra up to Noosa and within the Hinterland.
Key Responsibilities
- Conduct client assessments and develop care plans with clients, representatives, and stakeholders
- Update care plans regularly and conduct home visits
- Monitor clinical risks and client needs, ensuring appropriate referral pathways are in place
- Work collaboratively with internal teams, including Rostering and Practice Leads, to meet client needs
- Develop strategies and/or broker services to address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
- Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience
- Demonstrated Aged Care clinical case management experience
- Understanding of Aged Care funding and service delivery
- Confidence in utilising a variety of technology including mobile devices and computers
- Proficiency with Microsoft Office, Outlook and database systems
- Ability to effectively build relationships with diverse stakeholders
- Strong communication skills
- Ability to multi-task and work autonomously in a fast-paced environment
- A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
