About the Role

As a Case Manager – Youth Housing and Reintegration Service (YHARS), you will support young people aged 12–21 years who are experiencing homelessness or are at risk of homelessness.

You will work with young people who may be transitioning from Out of Home Care, exiting Youth Detention, or living in unstable or temporary housing. Using a trauma-informed, strengths-based approach, you will support young people to secure housing, build life skills, reconnect with family and community, and transition toward safe and sustainable independence.

This role works within the CARE model and requires strong relationship-building, collaboration and advocacy to achieve positive outcomes for young people.

This role is full-time, maximum term until 30 June 2026, based in Toowoomba.

Key Responsibilities

  • Provide trauma-informed, person-centred case management and brokerage support to young people experiencing or at risk of homelessness.
  • Apply CARE model principles, Motivational Interviewing and Therapeutic Crisis Intervention to support safety, behaviour regulation and goal achievement.
  • Support young people to access and maintain housing, education, training and employment.
  • Develop, implement and review individual goal plans and transition plans.
  • Actively engage young people in decision-making and planning processes.
  • Work collaboratively with Child Safety, Youth Justice and community stakeholders.
  • Support cultural safety and identity, including the development of Cultural Support Plans where required.
  • Maintain accurate records, reports and documentation in line with organisational and funding requirements.

What You’ll Bring

  • Tertiary qualification in Social Work, Psychology, Human Services or a related field (or equivalent experience).
  • Current Queensland Driver’s Licence.
  • Experience providing case management to young people with complex needs.
  • Strong understanding of homelessness, trauma and transitions from Out of Home Care or Youth Detention.
  • Ability to manage competing priorities while maintaining quality documentation.
  • Strong communication, engagement and relationship-building skills.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
