About the role
Assist NDIS participants in coordinating and implementing support services based on their NDIS plan, using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery. This is a permanent part time (4 days per week or shorter days over 5 days) position that can be located anywhere on the north side of Brisbane (including up to Redcliffe). You will have the flexibility to work from home but will be required to meet with clients as required and attend the Milton office once a week.
Key responsibilities
- Strengthen and enhance our client's ability to coordinate supports and participate in the community
- Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
- Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
- Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
Skills & experience
- Experience as an NDIS Support Coordinator
- Unrestricted drivers licence and willingness to travel as required
- Excellent verbal communication skills
- Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
- Established time management, problem solving and conflict management skills
- Ability to think creatively and use initiative
Successful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and a Blue Card.
