About the role
The purpose of a Hub Home Provider is to provide support services to satellite families participating in Life Without Barrier's Mockingbird Family Constellation.
The incumbent is key in the development of a healthy community of foster and kinship carers focused on the health, safety, growth and permanence of participating children and young people. This will include providing support to foster and kinship carers while enabling self-care that promotes healthy retention of carers.
The incumbent resides in a house that is conducive and appropriate to provide support to Satellite families participating in the constellation.
This is a Maximum Term Part Time Position until 30th June 2026
Key Responsibilities
- Organise, coordinate and host a monthly Constellation meeting at the Hub Home or another suitable venue, to include a meal, review of the Constellation calendar, other Constellation business as needed, and may include training or a guest speaker.
- Organise, coordinate and host, at a minimum, a quarterly social outing in the community for all satellite carers and children/young people.
- Host meetings at the home when possible, such as case planning meetings, care team meetings, as well as sibling and/or family visits (where appropriate).
- Record accurate information of monthly services provided, children and young people’s engagement in the Constellation and foster family recruitment activities. As required, maintain additional records as required.
- As required, maintain key information for carers and client’s records
- Maintain regular contact with Satellite families and children/young people, including telephone calls and communication regarding current placement issues, problem solving, identifying community resources, locating and correcting possible system errors.
- Provide help to Satellite families in developing plans for children’s success in school, identifying mental health resources and accessing other community-based services.
Skills & Experience
- Meet all requirements and successfully obtain, and maintain, authorisation/registration to act as a foster carer under the relevant jurisdiction
- Significant experience as a foster/kinship carer (5 years or more preferred, or comparable experience); or employed experience in the Child Protection or Out of Home Care sector
- Possess and demonstrate the emotional energy, maturity, skills and experience to provide support to children and young people in the Constellation.
- Possess and demonstrate capacity to engage in appropriate relationship building with carers and children/young people within the context of the Hub Home Provider role.
- Current First Aid Certificate
- Current Driver’s licence.
