About the Role
Join our team as a Care Coordinator supporting Aged Care services in Rockhampton and surrounding areas. In this role, you’ll manage referrals, onboard new clients, and adapt care plans to meet changing needs. You’ll coordinate home visits and work closely with clients and their families to deliver person-centred support.
You will help to improve day-to-day operations by refining systems and ensuring high-quality service delivery. Your work will ensure clients feel supported and valued while contributing to the ongoing growth of our aged care programs.
We have multiple Full Time positions available based in our Rockhampton office, with some travel required to clients' homes across the region, including Yeppoon. This is a rewarding opportunity for someone with Aged Care experience that is passionate about making a meaningful difference in the lives of older Australians.
Key Responsibilities
- Undertake comprehensive client assessments to develop client profiles and care plans in collaboration with the client/representative and other relevant stakeholders
- Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits
- Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients
- Collaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering, Practice and Workforce teams
- Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
- Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience
- Demonstrated Aged Care or other clinical case management experience
- Understanding of Aged Care funding and service delivery
- Confidence in utilising a variety of technology including mobile devices and computers
- Proficiency with Microsoft Office, Outlook and database systems
- Ability to effectively build relationships with diverse stakeholders
- Strong communication skills and confidence in delivering training
- Ability to multi-task and work autonomously in a fast paced environment
- A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
