About the role

Assist NDIS participants in coordinating and implementing support services based on their NDIS plan, using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery. 

This role is permanent part time (Flexible 3 or 4 days per week) which will support people based primarily in the Dandenong area and so, can be flexibly based in Dandenong or surrounding suburbs of Melbourne with hybrid work from home arrangements.

Key responsibilities

  • Strengthen and enhance our client's ability to coordinate supports and participate in the community
  • Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
  • Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
  • Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency 

Skills & experience

  • Demonstrated experience in working with people with a disability and mental health issues
  • Experience in a coordination role 
  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
  • Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
  • Established time management, problem solving and conflict management skill
  • Ability to think creatively and use initiative

Successful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Children Check.
