About the role 

In this role you will be responsible for developing and implementing therapeutic case plans for the individual needs and goals of children and young people in our Therapeutic Supported Independent Living (TSIL), Supported Independent Living (SIL) and Case Coordination – Not in placement (CC-NIP) programs in the Ballina region. 

You will play a key role in working towards and building the young people’s capacity for independence and their transition out of care. The successful candidate will have experience working with young people in out of home care or family-based care, previous experience managing complex caseloads and an understanding of trauma and its effects on children and young people.

This is a permanent part time position will be required to work 4 days per week (Monday, Tuesday, Thursday, Friday). The role involves regular travel within the Ballina and a company vehicle will be provided for this.  

Key Responsibilities  

  • Develop and implement high quality therapeutic case plans for Children and Young People in LWB's care 
  • Develop and maintain functional and supportive relationships with children and young people in therapeutic care - Significant Disability as well as their families to support positive outcomes
  • Attend shifts within the intensive therapeutic care homes, and work closely with the team 
  • Meet with clients and family members to gather input for case plans and ensure their views are represented 
  • Coordinate and network with other agencies to effectively achieve case plan goals within tight time frames.
  • Participate in an on-call roster as required 
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery 

Skills & Experience 

  • Relevant bachelor’s degree, or a relevant diploma and be working towards a degree. (A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience and a willingness to undertake further training)
  • Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting
  • Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.
  • Understanding of current casework practices for children and young people 
  • Understanding of trauma and its effects on children and young people
  • Current  drivers’ licence
