Role: Case Manager – Youth Housing
Location: Lockyer Valley (Gatton/Laidley and surrounds)
Work Arrangement: Work from home/client locations with access to office in Ipswich & Toowoomba
Employment Type: Maximum Term, Full Time (2 year contract)
In this role, you will provide outreach and case management support to young people who are homeless or at risk of homelessness, helping them access safe housing and essential community services. You will work with young people to develop practical goals and support plans that build stability and independence.
The role involves engaging in assertive outreach to find young people in need of support in the community across a range of public spaces. You will also collaborate with housing providers, government agencies and community organisations to coordinate support, using a trauma informed and culturally safe approach to build trust and support positive long term outcomes.

Key Responsibilities:
- Provide outreach to young people who are homeless or at risk of homelessness, helping them connect with appropriate supports.
- Manage a caseload and work with young people to develop practical goals and individual support plans.
- Assist young people to secure and maintain safe and stable housing.
- Support access to education, employment, health and community services to build independence.
- Work collaboratively with stakeholders such as housing providers, government agencies and community organisations to coordinate support and achieve positive outcomes.
- Use a trauma informed and culturally safe approach to build trusting relationships and support young people to achieve their goals.
Skills & Experience
- Relevant tertiary qualifications within the community service sector and/or relevant experience.
- Demonstrated experience in outreach and case management with young people experiencing complex needs or homelessness.
- Strong ability to engage and build rapport with vulnerable young people.
- Sound understanding of trauma informed practice and the impacts of homelessness and disadvantage.
- Ability to work within multidisciplinary teams and navigate community and government systems.
- Strong organisational, problem solving and risk assessment skills, with the ability to work independently and manage competing priorities.
- Current Queensland Drivers Licence
Successful candidates will be required to clear probity checks including National Criminal History Record Check and any relevant stated based checks.
