About the Role
As a Child and Family Practitioner, you'll provide high-quality case management and support to children and young people living in family-based care arrangements.
Working collaboratively with foster carers, Child Safety, families and community stakeholders, you'll develop and implement individualised care plans, strengthen placement stability and support positive outcomes for children and young people.
Based in our Mackay office, this is a permanent full-time opportunity offering meaningful work, flexibility and ongoing professional development.
Key Responsibilities
- Deliver child-focused case management, planning and assessments.
- Build strong partnerships with foster carers, Child Safety, families and other stakeholders.
- Provide ongoing support, supervision and guidance to Foster Carers through home visits and regular contact.
- Support carers to implement case plans and achieve positive outcomes for children and young people.
- Develop and monitor risk assessments, cultural support plans, safety plans and behaviour support plans.
- Maintain accurate and timely records, case notes, foster care agreements and renewals.
- Contribute to a collaborative, child-focused team environment.
About You
You'll be passionate about improving outcomes for children and young people and confident working collaboratively with carers, families and key stakeholders.
You'll also bring:
- A recognised degree in Social Work, Psychology or a related discipline
- Experience within child protection and/or out-of-home care
- Demonstrated ability to work from a strengths-based, trauma-informed and culturally responsive approach
- Experience supporting foster carers and managing complex casework
- Excellent communication, organisational and relationship-building skills
- Current Australian Driver Licence
Successful candidates will be required to complete probity checks, including a National Criminal History Check and Working with Children Check (where applicable).
