About the Role
The Case Manager – Next Step Plus/Extended Post Care (NSP/EPCS) supports young people aged 15+ as they transition from out-of-home care to independent adulthood. Using a culturally responsive, person-centred approach, you will empower young people to build the skills, confidence, and connections needed to thrive, with ongoing support available up to age 25.
Working within a care team, you will address the impacts of trauma while providing practical support, advocacy, and guidance. This role requires someone who is compassionate, organised, and confident managing a complex caseload, balancing direct client work with strong administrative and compliance requirements.
This is a Permanent Full-Time position based in Ipswich.
Key Responsibilities
- Deliver person-centred case management to support young people transitioning to independence
- Develop and implement transition, financial, and cultural support plans
- Apply Therapeutic Crisis Intervention (TCI) strategies to support behaviour and coping skills
- Collaborate with Child Safety, families, and service providers to achieve positive outcomes
- Facilitate connections to community, culture, education, housing, and employment supports
- Support engagement with Aboriginal and Torres Strait Islander communities
- Maintain accurate records and meet reporting and compliance requirements
About You
- Relevant degree in Social Work, Psychology, Human Services or similar (or equivalent experience)
- Experience working with young people with complex needs, ideally in out-of-home care or post-care
- Strong case management, engagement, and advocacy skills
- Ability to manage competing priorities and complex caseloads
- Understanding of trauma-informed practice and transition challenges for young people leaving care
- Experience supporting Aboriginal and Torres Strait Islander young people and communities (desirable)
- Current QLD Driver’s Licence
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
