About the Role
As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Mackay. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering the growth of our aged care services.
This is a Permanent Full Time opportunity based in our Mackay office. Some travel will be required to clients' homes across the region as needed.
Key Responsibilities
- Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders
- Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits
- Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients
- Collaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Practice Leads
- Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
- Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience
- Demonstrated Aged Care or other clinical case management experience
- Understanding of Home Care Packages, Commonwealth Home Support Program, Aged Care services and My Aged Care Standards
- High level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systems
- Ability to identify and implement risk mitigation strategies
- Strong communication skills to build relationships with diverse stakeholders
- Ability to multi-task and work autonomously in a fast-paced environment
- A current unrestricted Australian Driver Licence and reliable vehicle
