Life Without Barriers’ Synergy Youth and Family Mental Health Service provides intensive support to young people aged 12 to 25 who are experiencing severe and complex mental health challenges. Our team of clinicians and case managers provides comprehensive clinical assessment and formulation.
Support is offered in community settings, such as the young person’s home or other local community environments, allowing therapeutic work to take place in real time and within the context of their daily life. The service also helps young people develop their functional skills and daily living abilities while working closely with them and their support network for up to 12 months.
Role: Clinician (Mental Health)

In this role, you will work one-on-one with young people and their families, supporting them to improve their mental health, well-being, and overall functioning while walking alongside them as they achieve their goals.
As part of a skilled, supportive wrap-around outreach mental health team, you will use your expertise to conduct assessments, develop care plans, and deliver therapeutic interventions and case management. You will provide outcome-focused support to young people and families with complex needs, helping them achieve meaningful, positive results.
Key Responsibilities
- Develop comprehensive formulations, care plans and risk assessments to inform appropriate intervention.
- Administer, interpret and report on a range of client assessments as appropriate (consistent with professional registrations and client need).
- Provide a range of evidence-based psychological interventions to individuals/groups presenting with multiple needs.
- Monitor and review clients’ progress against the care plan and outcome measures.
- Engage in case discussion and review with partner organisations and care team, demonstrating client outcomes.
- Through intervention and clinical case management, and through linkage to support services, support clients to maintain or re-engage in education and/or employment, social, and recreational networks, self-management, independent living, and increase individual and community-based relationships
- Work collaboratively with the client and the care team to plan for and develop the skills to exit or transition from the service, including referral to other services where appropriate.
Qualifications & Experience
- Tertiary qualification in a relevant Allied Health field (e.g., Psychology, Social Work) with 2–5 years post-graduate clinical mental health experience.
- Experience providing therapeutic support to young people (12–25) and their families.
- Ability to work both autonomously in the community and collaboratively within a multidisciplinary team.
- Skilled in clinical assessment, care planning, and delivering evidence-based psychological interventions to young people and families.
- Experienced in collaborating with agencies supporting clients with complex needs, including mental health services, GPs, psychiatrists, and other statutory or community services.
- Knowledge of and experience working within internal and external Clinical Governance frameworks.
- Current registration or eligibility with the relevant professional body.
- Committed to youth-friendly, client-focused care and delivering positive outcomes through clinical case management and psychological interventions.
- Flexible and willing to provide outreach, e-health, and community-based interventions using a recovery-oriented approach.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Tasmanian Working with Vulnerable People Registration.
