About the role
Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach. You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers.
This role is a permanent full time or permanent part time (3 or 4 days per week) opportunity where you will have the flexibility to work from home. There will be some travel required to meet with clients across Adelaide so an SA driver’s licence and your own reliable vehicle is required.
Key responsibilities
- Strengthen and enhance our client's ability to coordinate supports and participate in the community
- Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
- Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
- Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
Skills & experience
- Experience in Support Coordination or Disability Employment Services role
- The ability to work in a KPI driven environment where motivation and commitment are essential
- Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
- Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
- Ability to work autonomously and use initiative
Successful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and SA Working with Children Check.
